Application for Water Service Connection

Who may apply:

1.) Owner of the household.

2.) Lease or any authorized representative.(bring authorization letter)

FEES:

Registration Fee - PHP 500.00 /connection
Tapping Fee - PHP 200.00 /connection
Service Fee - PHP 150.00 for the first(1st) faucet & PhP 100.00 for every succeeding faucet.
Connection Line(P.E. tube) - PHP 3.00 /linear meter when P.E. tube is not from the WD office.
Connection Line (G.I. Pipe) - PHP 50.00 /length

Note: Materials after the stub-out shall be shouldered by the concessionaire.

REQUIREMENTS

A letter of consent from the lot owner , if the concessionaire's lateral line will traverse/pass a lot not owned by the applicant.

Other Fees:

• Copy of bacte-testing/PCHEM result - Php 100.00 / copy

• CERTIFICATION - Php 50.00/copy
